Application For OKU Stickers

Conditions of Application

a. Applications may be submitted by the registered owner, beneficiary or representative of the organization/ association.

b. Vehicle registered under the name of the disabled persons possessing OKU Card with reference number that starts with ' PH ' (disabled persons with physical disability) or ' CA ' (old OKU Card reference number issued by the Social Welfare Department in Sarawak) or;

c. the vehicle has been registered under code uses IT (vehicles for disable people) or;

d. Vehicle owned by organization / association of the disabled who have registered /received confirmation from the Department of Welfare.

Documents needed

Application by the registered vehicle owner

a. 1 JPJK17 Form

b. Original owner identification

c. Original OKU registration card or a copy if application is made by the beneficiary/representative.

Application by the beneficiary

a. 1 JPJK17 Form

b. Original beneficiary identification

c. A copy of the OKU registration card

d. A confirmation letter from the Social Welfare Department of Malaysia

Application by organizations /Association for the Disabled

a. 1 JPJK17 Form

b. Original representative identification

c. A confirmation letter confirmation for organization / association of the disabled who have registered with the Social Welfare Department of Malaysia
